The form will take a few minutes to complete and you will need your identification documents.
After filling out the online application form, there are two other admission conditions:
1. Send the required paperwork listed below to the Study Division
2. Pay tuition of 110 000 CZK by October 31st 2025 (must be received in full amount, be aware of possible exchange rates and bank fees)
Once these conditions have been met, you will receive a statement of acceptance for the Premedical course.

Medical report
Medical report certify that you are physically and mentally fit to start the course. Print the report and have it filled out by your doctor.
